Rembrandt Theme - how to edit Subscription widget?

Hi,
I have signed up with aweber for my Subscription form.

1. Currently the widget on the footer only allows for my customers to key in their email address only.
I want to insert another row for them to key in their First Name. How do I insert this?
Without knowing their first name, will make my emails to them very impersonal.

2. Also, from aweber, I can create different design of the Subscription widget. How do i insert my customized Subscription widget into my Rembrant theme?

Go to Customization / Subscribe Form ... replace to code there with your code.

I'm assuming that aweber offers a html form of the subscribe box. (e.g ... <form> blah blah blah blah </form> )
