New Checkout look wrong

@Sergei Alidropship please see the above second image .

This is the new checkout I got when I downloaded a theme and install it. I don’t like this layout because is gave me issue in custom field and I personally I believe is confusing the customers.

I want the checkout that was always is the demo theme you had in the alidropship theme section, see the first images. How can I achieve that ?

Is a clean fresh installation only woocommerce and Da Vinci theme

UPDATED: i deleted the html code of the page and added the shortcode of woocommerce page, which fixed the issue.
